To increase the contribution amount to your retirement plan, complete the appropriate Salary Reduction Agreement (SRA) below and return it to the person in your organization who manages the payroll.  Please keep a copy for your records. You do not need to return a copy to GuideStone.

Standard SRA  — This form should be used by all GuideStone participants with the exception of:

• Baptist Memorial Health Care Corporation (BMHCC) participants — BMHCC SRA
• International Mission Board (IMB) participants —  IMB SRA
• Baylor University participants — Visit the Baylor University Compensation and Benefits site for the correct SRA.
